STA No 204 Denosumab for the prevention of osteoporotic fractures

NHSScotland should note that:

1. Recommendations of NICE Single Technology Appraisals (STAs) have no status in NHSScotland.

2. NHS boards should adhere to the Scottish Medicines Consortium (SMC) advice.

SMC published a Statement of Advice (651/10) on this medication for this indication in December 2010. This stated that:

Denosumab 60mg solution for injection in a pre-filled syringe (Prolia®) is accepted for restricted use in NHSScotland for the treatment of osteoporosis in postmenopausal women at increased risk of fractures. It is restricted to patients with a bone mineral density (BMD) T-score < -2.5 and ≥ -4.0 for whom oral bisphosphonates are unsuitable due to contraindication, intolerance or inability to comply with the special administration instructions.

3. There is a material difference between the recommendations of the NICE STA and SMC. The NICE STA restriction is based on age and number of independent clinical risk factors as well as BMD, for primary prevention. For secondary prevention there is no difference between the recommendations.
